It's surprising how often we humans think that what is crystal clear inside our own mind is just as clear to everybody else.

We meet CEOs all the time who fall into this trap. They know exactly what needs to be done. Their employees don't have a clue.

It's not bad will from either side. It's just easy to forget that having something clear in your own head doesn't mean you've actually communicated it in a way that sticks. Sometimes you've said it so many times you're sick of hearing yourself say it, and people still haven't understood what you mean.
